  3. Certificate of Limited Partnership form LP 201 (LP)
  4. Statement of Qualification form UPA-1001 (LLP)
  5. Partnership Agreement

Non-Profit Organizations

  1. Tax ID letter from IRS
  2. Article of Incorporation, if applicable
  3. By-Laws
  4. Proof of tax exempt status (501-C3)
  5. Meeting Minutes

Corporation
